Overview

Results with lasers in the past 20 years have been dramatic. Lasers have been in use since the 1950’s but it was only in the 1980’s when huge steps forward were made, making them the gold standard for other treatments to try and live up to.

What is it?

From science fiction films we derive fantastic ideas of what lasers can do. If it’s not Captain Kirk stunning a Cling-on, it is James Bond watching the laser burn through the sheet metal table he is tied to!

Medical lasers are a far cry from such lasers. Lasers emit light, that’s all. It’s very similar to the light coming from your ceiling light bulb, but not the same. The science of lasers is fascinating, but without getting too complicated it’s enough to know that laser light has several unique properties that distinguish it from natural sunlight or electric light.

One such critical property is its single wavelength. When the correct laser light lands on the target tissue, the target (e.g. facial thread vein) recognises that wavelength as its match. Different tissues absorb laser light differently depending largely on wavelength. The laser light energy is thus absorbed by the target tissue but not the neighbouring tissues that cannot absorb that wavelength. This extremely precise targeting means the target tissue (e.g. facial thread vein) can be removed without damage to surrounding tissues.

Side effects (see below) are very much reduced and importantly this makes lasers the most effective, and the safest of all available light based treatments.

Uses

  • Rosacea (see www.dermnetnz.org for details) and facial thread veins with or without generalised redness
  • Brown liver or age spots (solar lentigo, see www.dermnetnz.org for details)
  • Red spots and dots (Campbell de Morgan spots, see www.dermnetnz.org for details)
  • Tattoo removal
  • Port Wine Stain birthmarks (see www.dermnetnz.org for details )

Safety

Long term side effects with lasers are rare in experienced hands. This is because of its extraordinary ability to target specific tissues without affecting neighbouring structures (see above). Scarring is highly unlikely. In darker skin types lightening of the skin can occur but your doctor will discuss this with you in detail at the consultation when he or she examines you.

Unwanted effects vary depending on which laser is used. Occasional crusting is seen the day after treatment. They rapidly disappear.

In tattoo removal treatment occasionally causes pin point bleeding initially. This could be avoided at future treatments if desired but it may take more treatments to achieve satisfactory results. The tattoo may be covered afterwards. As above tiny crusts may form as well but they rapidly disappear.

In port wine stains, and sometimes in generalised redness of the face (e.g. Rosacea) will leave temporary circular marks after treatment but this can be minimised with cold air colling that is applied at the same time as the laser. If necessary make-up may be applied immediately after treatment in the clinic. They disappear within a few days.

Darker patches can develop if laser treated areas are exposed to sunlight. Your doctor will emphasise to you the importance of sun protection for treated areas for 3-6 months after your last treatment. This means covering up the treated area with clothing or sun cream, factor 30 or higher. This does not mean you cannot have treatment in summer or you cannot go on holiday, it means that you must use sun protection. If sun protection is ignored and darker patches develop they will disappear, but they may take a number of months to go.

The Client’s Experience

You will be seen throughout by an experienced doctor. At the first visit you will be asked about your concerns and your goals of treatment. A brief medical history will be taken. You will be given as much explanation and time as you need which means at youngerfaces you will always be in the driving seat. You choose your treatment after a thorough discussion of all the options. An explanation of the proposed lasers and the treatment is always given before proceeding.

Treatment with a laser at youngerfaces is not regarded by clients as painful although you will be aware of something, usually a mild discomfort. It depends on which laser you have but it may for instance feel like the flick of a rubber band. Many treatments are over in a few minutes but some, like port wine stain birthmarks, take longer.

You would normally be seen for several treatments, each visit around 8 weeks apart to achieve good results. The number of visits may be 1-2 for red dots and spots, 2-4 for brown liver spots, or 4-6 for facial thread veins. These are not hard and fast guidelines ,just typical figures for other clients.

Results

Rosacea (see www.dermnetnz.org for details) and facial thread veins with or without generalised redness

Pros

  • Effective
  • Very safe with a extensive track record of safety data from hundreds of published studies in peer reviewed quality international medical journals over past 30 years
  • Quick
  • Long lasting results: permanent improvement is expected
  • Non-surgical

Cons

  • Unwanted effects (as above) but nearly always rapidly disappear

The Future

As above the benefits of laser are long lasting. The results seen are generally permanent. In some conditions the problem may recur e.g. new facial thread veins may appear, but the old laser treated ones do not come back.
